Some records from 1969 to present are kept at the hospital,  however, they are not easily obtained. You may write or call Medical Records at the above address/phone number.    

You will need patient's full name, Social Security Number, and a letter stating why you want the records.  If the patient is living, you will not be able to obtain the records without patient consent.
